Adds the documentation, issue templates, and legacy AUTHORS archive for the Flutter Contributor Ladder:

• Adds contributor ladder documentation under docs/ (LADDER.md, GOVERNANCE.md, QUARTERLY_REVIEW.md, CODE_REVIEW_WORKFLOW.md, TRANSITION.md) and updates README.md, CONTRIBUTING.md, and CONTRIBUTORS.
• Creates archive directory for AUTHORS files under legacy_authors/ (flutter.AUTHORS, packages.AUTHORS, devtools.AUTHORS, website.AUTHORS).
• Adds GitHub issue templates under .github/ISSUE_TEMPLATE/ for out-of-band nominations, Dasher Emeritus reinstatement, quarterly review tracking, and partner verification.

Do out-of-band and Dasher Emeritus need to be two separate forms?

Good question. With the initial launch, many many contributors will be moved to emeritus. They and others on the team expressed concern that they felt like they would have to "start over again", and asked for a special re-entry point. Hence the separate special template to recognize their return.
